Attendees will learn how Senior Care Partners PACE supports seniors through coordinated medical, social, and in-home care services, and how this approach reduces hospital visits, supports caregivers and strengthens communities. The session will also explore how local businesses, civic groups, and service partners can collaborate with Senior Care Partners PACE to enhance community well-being, create referral pathways, and support aging with dignity and choice.
